Instructions

1. Download the appropriate Project or Thesis registration form for your program.
   • CHG Thesis
     • CHG 4901 (PDF, 140 KB)
     • CHG 4902 (PDF, 140 KB)
   • MCG Thesis - MCG4100 / MCG4220 (PDF, 31.7KB)
   • SEG Project - SEG3904 (PDF, 31.2KB)
2. Clearly complete all student information sections and obtain the required signature(s).
3. Send the request with the necessary documents at the Undergraduate Studies office in person at room 1020 at the SITE building or by e-mail at [email protected].

The Faculty of Engineering reserves the right to return any incomplete form.
