To report an accident or illness that occurred during your placement hours, you are required to complete the steps below. The student, placement employer, and the placement coordinator all have duties and responsibilities to ensure that the necessary requirements are fulfilled.

To report an accident or illness that occurred during your placement hours, you are required to complete the following steps:

  1. IMMEDIATELY report the injury or exposure to:

    • Your clinical or placement supervisor

    • The appropriate hospital contact (see contact information below)

    • Your liaison officer (UGME) or program administrator (PGME)

  2. Report to the nearest ER for treatment (unless instructed otherwise by hospital Occupational Health or Learner Experience teams).

  3. Complete the uOttawa online Accident, Incident, Occupational Disease or Near Miss Form.
