Innovative Health Systems & Tailored Models of Care

Care for those at risk of, or living with, brain-heart interconnected conditions (BHICs) is fragmented. Care delivery should consider jointly the elements of both the BHICs. We are establishing innovative strategies for the care of BHICs that regions can apply to address gaps in their existing care delivery system. We are also establishing an approach for knowledge exchange with communities to support the adoption of evidence-based strategies and wise practices for improved BHICs care delivery. Our goal is to create knowledge that will be scaled up broadly and approaches that can adapted for various BHICs.
Deliverables: We apply a patient-oriented research approach in the development of innovations to ensure the relevance and applicability of our work. We also use a learning health system approach in our implementation work, where we consider new evidence and findings emerging from the work, the changes we introduced to inform ongoing adaptations of the strategies implemented and optimize its impact. Equity-seeking populations, including some Indigenous communities, will be engaged to compile an evidence map of the most effective approaches for tailoring these new models of care. This will foster ethical and culturally responsive collaborations that honour Indigenous ways of knowing and support these communities in building and sustaining resilience. The knowledge we generate will support the broader application of our work.
