How to Apply
*Important note before getting started: Please make sure to download the application form for group learning and supporting documentation from our website, or contact us should you be unsure of what forms to download, as they are updated frequently. Using an old Application form, or old Conflict of Interest form, for instance, will delay the processing and reviewing of your application.
- Review the requirements for accreditation of a program that are described in the application form for group learning;
- Fill in the application;
- Attach all the required documentation and review the Checklist (.docx 51.32 KB) before submitting;
- Email the completed application and supporting documentation to: [email protected]
If you are running into difficulties, please let us know, and perhaps we can guide you and save you some time.
- For CFPC Programs, you will need to complete the Statement of Involvement Form in program planning.

Accreditation Review Fees
Please contact our office for information about our accreditation fees. Accreditation review fees may vary depending on the length and complexity of the review. Additionally, programs with revenue are charged at a higher rate than events not receiving revenue. Revenue includes registration fees and/or sponsorship. Money in the form of departmental funding does not constitute revenue.
Accreditation fees cover the cost of the review and are non-refundable regardless of the outcome of the review, and are subject to HST except for uOttawa departments.
